Dental implant treatment is considered as the best treatment modality in replacing missing teeth.Dental implant acts as a root and on top of it cap is fixed.  It is very important to have sufficient length and width of implant to hold cap (Crown) and also to withstand the chewing forces.

It is like having a strong foundation deeper into the ground to hold a building in case of construction of house on top of it. But to place such good length of implant,it is also very important to have such good amount of bone. If there is no sufficient bone, we can’t place a good size implant.

A minimum, good size of dental implant to withstand chewing forces is 8 mm – 10 mm. But if bone is less than that, we have to do surgical procedures to gain that much bone.

A sinus lift procedure is essentially done to increase bone height and density in the posterior maxilla (Upper back teeth region) to place implants. There are two techniques such as direct and indirect ways.

Indirect sinus lift technique:

It is also called as Osteotome technique/ Crestal approach / Transalveolar approach etc.It is extremely effective in increasing bone height to place implants.

This procedure requires, minimum available bone 5-6 mm existing bone and we can gain another 5-6 mm bone height to put implant size of 10-12 mm in size.

Less invasive procedure than direct sinus lift

No need of aggressive surgical intervention

Minimal skill is required

Procedure is performed with simultaneous implant placement

Damage to the sinus is minimal without affecting the sinus pressure

Procedure:

Dr.Sudhakar Reddy  thoroughly pre-evaluates the implant site with detailed radiographic examinations. The hard and soft tissues are fully inspected and diagnostic information collected before the surgery.

Measurements are carried out for:

Alveolar height to sinus floor

Mesio-distal and bucco-lingual width of the edentulous space using IOPA (intra-oral peri-apical), CT dentascans and OPG (orthopantamogram) x rays

Local anesthesia (infiltration) may be done with 2% lignocaine

2 -3 mm incision is made on the palatal side of the ridge crest andsome time, it can be done as a key hole technique

Full-thickness mucoperiosteal flap is reflected and retracted

Implant position is marked with a round bur

Pilot drill is used to prepare implant bed (1-2 mm short of sinus floor)

The sinus floor is broken with an osteotome

The bone mobilized at the sinus floor is raised with the osteotome

The bone graft can be used in necessary cases to raise the membrane.

The implant is inserted at the site prepared for it

Titanium cover screw is inserted on the implant

The mucoperiosteal flap is repositioned and sutured
